To International patients seeking medical care in IUHW Mita hospital
Our role as the medical institute
- IUHW Mita Hospital is the medical institution catering to patients who require acute-phase treatments.

If your condition is not severe, visit a nearby clinic first. When a clinic doctor decides that you need specialized treatments, you should consider visiting IUHW Mita Hospital with a referral letter.
Without the referral letter from another medical institution, you will be asked to pay 5,500 JPY as a "fee for treatment of patients' choice", in addition to the first visit fee - When your condition becomes stable or takes a chronic course after inpatient treatment, you need to leave the hospital or be transferred to a recovery/chronic ward in other hospital
- IUHW Mita Hospital has no obstetrics (maternity) department. Psychiatry and pediatrics inpatient treatments are also unavailable.
Procedures for outpatient consultations
Please make sure to bring your "Medication Record Booklet” (Okusuri Techo), if applicable.
For first-time patients

Reception area (1F)
Fill out “Registration Form” first. Then, visit the reception with completed Registration Form, Health Insurance Card and if any, a referral letter (If you have the Medical Care Certificate for infants/the disabled, please submit as well). You will receive the patient ID card, a registration sheet and a number ticket. Keep them all into your clear plastic folder and head for the department desk where you need a consultation. -
Department desk (1~4F)
Submit the clear plastic folder at the department desk. Then, you will be asked to complete a medical questionnaire and turn it in the desk. When your turn comes, your number is called or displayed on the screen in the waiting room. -
Medical consultation room
Your name and birth date are asked before the consultation starts. -
Various medical examinations and treatments
When it is necessary, medical examinations such as blood collection, urinalysis and X-ray may be performed in respective test rooms. -
Medical consultation room
The doctor diagnoses your condition based on the examination results. At this stage, the doctor may order additional examinations, including CT or MRI. If it is not urgent, such examinations are set at a later date.

Reception area (1F)
Please check in with the automated registration machine for returning patients. The machine prints out a registration sheet and a number ticket for you. Please put them into a clear plastic folder and head for the department desk where you need a consultation. The Health Insurance Card need to be checked at the beginning of a month; come to the reception, if applicable. -
Department Desk (1~4F)
Submit the clear plastic folder at the department desk. When your turn comes, your number is called or displayed on the screen in the waiting room.
Make sure to come to the department desk by the appointment time. Doctor's consultations can be ahead of/behind the schedule, depending on the situation. If the patient arrives behind the appointment time, doctor's consultations may not be provided.
※ Precautions
- Submit the plastic folder first at the department desk where you see a doctor, then wait there until your turn comes.
- In some departments, patients need to wait longer.
- Each treatment requires different time frame, which means that the order of seeing the doctor does not always corresponds to the reception number.
- Patients without appointment can be examined in between the appointed consultations. Emergency treatment is prioritized, regardless of the order of appointment.
- The necessities of medical prescription/examinations are determined by doctors. When any medical examination or medication is requested at the patient's discretion, please consider the use of comprehensive medical checkups (Ningen Dock) or pharmacy-selling over-the-counter (OTC) products.
Important notice for medical consultation

When having no request in which department to see a doctor, let us know the most suffering symptom.
If the symptoms are not reported correctly, it would take more time to refer the patient to the appropriate department. - Arrange a professional medical interpreter in advance, if the patient has no good command of Japanese.
Doctors make diagnoses through the patient's words, if any misunderstanding occurs along the way, it could lead to a misdiagnosis. The use of specially-trained professional medical interpreter is indispensable. If there is no arrangement information, contact the reception. - Medical consultation with a non-professional interpreter (patients' family members/relatives/company colleagues who understand Japanese) is not recommended. The intervention of non-professional interpreter may bring various disadvantages during the course of patient medical care. In hospital setting, unfamiliar medical terms are frequently used, which requires sufficient expertise for accurate interpretation. In addition, it would pose psychological burden on a person who needs to function as a communicator without professional training. Furthermore, exposure of patient disease information may sometimes compromise the interests of patient.
- For returning patients: when cancellation is needed, please be sure to call us before the scheduled appointment day.
- Bring your "prescription record booklet" (Okusuri Techō) without fail, when visiting the hospital.
- When unfamiliar words are heard, please ask us anytime; difficult medical terms during the consultation can be paraphrased in a more plain expression.
- Vital information (issue what the patient thinks is important for the treatment) should be repeatedly confirmed to the medical staff by the patient.
- When receiving informed consent form (for tests/treatments), read it thoroughly. Only after totally agreeing with the content, the patient can sign the form.
Precautions for examinations
- In some medical examinations, diet restrictions can be set from the eve of the test day, with strict time schedule. If it is not followed as instructed, the test may not be performed in some cases.
- Metallic accessories should be removed in X-ray/*MRI scan. When the patient is wearing tattoos, let us know in advance.* Magnetic Resonance Imaging Scan
- Be sure not to drive after the test, when it is instructed.
Payment at the cashier
- Medical treatment costs should be paid at the cashier. When the patient has prescription, a voucher for the prescription is given with the receipt.
- Credit/Debit cards are available for payment at the cashier. In addition, China Union Pay Card are not available for payment.
- Without Japan’s Health Insurance coverage, patients need to pay 200 % of the total medical fee.
Receiving prescriptions

Patients' prescriptions are handed at the pharmacy counter (left next to the cashier).
The screen on upper left of the pharmacy counter shows a number whose prescription is ready. The number showed the biggest on the screen, is the most recently set prescription, meaning smaller numbers than that, prescriptions have already been set. - Pharmacists at the counter provide medication instructions: how to take/apply medications. Medication instructions in Japan are not always the same as those in other countries.
- Take/apply medications as directed; do not stop medications at your own discretion. If any question arises, please contact the doctor or pharmacists.
Admission information
- When hospitalization is needed, go through the procedure at the Admission Center. A minimum deposit of 100,000 JPY is required for admission.
- Read through the leaflet “Guide to Hospitalized Patient” to prepare for admission.
- Hospital facility for inpatients is called a “ward”. Each ward has the staff station, where medical staff, including nurses are on duty. If any question arises, please ask there.
- The Cancer Patient Support Center / Medical Consultation and Palliative Care Center offer various information. Please contact the reception on the 1st floor if you need assistance or consultation in issues, such as transfer to a different hospital, admission to a welfare facility after discharge and the cost of medical care.
- Mita Hospital has three types of hospital rooms: Private room, Triple (Three-bedded) room, and Quad (Four-bedded) room. Please be noted the hospital does not always accommodate room requests.
- Food intake can affect patient’s conditions and treatments. If there is any food gift for patients, please ask nurses whether or not it is allowed.
- Visiting a patient is allowed during designated visiting hours. Please go to the guard station at east entrance on the 1st floor first to make a procedure and receive a visitation permit.
- While visiting a patient, please refrain from talking in a loud voice. Also the use of cellular phone is prohibited in hospital rooms.
- Cellular phone is allowed to use in the lounge. Please talk quietly and finish it short.
- Mita Hospital has a store (on the 2nd floor) and a restaurant (on the 11th floor). The store offers food, beverages and various commodities, including towels, tissue papers, tooth paste/brushes, paper diapers and magazines.
- All the hospital staff (including your doctor and nurse) must graciously decline gratuities of any kind.
Dos and Don'ts in the hospital premises
- Covering natural facial/nail color can interfere with accurate diagnosis. Please refrain from wearing perfume and makeup.
- Those with sneeze/cough/fever should wear surgical masks in the hospital to prevent infections. Your cooperation would be appreciated.
- Do not speak loudly and wait quietly in the hospital public spaces such as hallways and waiting rooms, as with so many patients who feel sick out there.
- Do not run inside the hospital building. It will increase the fall risks among patients with a wheel chair/walker/cane/IV pole, which sometimes cause fracture.
- Smoking and drinking are prohibited in the hospital premises. When cigarette smoke is sensed in the hospital toilets, sprinklers there will go off.
- Do not spit on the hospital hallways.
- When using a toilet, use toilet paper equipped there and flush it in the toilet.
- Set your cellular phone in silent mode. Talking on the phone is allowed in the designated mobile-phone space on the 1st floor or outside of the building.
- Photographing/Video recording are prohibited in the hospital (except for a record sorely for your own use). Previous permission is necessary, when photographing/Video recording is needed.
- Mita Hospital declines to provide medical care for those with violent behaviors (i.e. using offensive language at hospital staff), which can undermine good mutual relationships. If the behavior is so extreme and out of control, we may report it to the police.
